How do I add or cancel an
employee?
You can add or cancel employees during open enrollment, or within 31 days of a
qualifying event. Qualifying events include marriage, divorce, birth of a child
or adoption of a child, loss of employment, new employment, or death of
insured.
All additions and cancellations will be effective on the first of the month
following any waiting periods. If an employee is cancelled in the middle of the
month, then the cancellation will take effect the 1st of the following month.
You can go on-line and add or delete employees and their dependents if the above
qualifying events and criteria are satisfied, or you can fill out the
add/cancel employee form which was given to at the time the contract was signed
and fax it to our office.
Do I have to administer the
plan or can I have someone else administer the plan?
Benchmark Dental Plans strongly recommends that you administer the plan or have
a third party administer the plan, since you are the best informed source of
when qualifying events, such as hiring and terminations, occur for your
employees.
We are a small size company
of less than 25 employees, what if we do not have the resources to administer
the plan?
Benchmark Dental Plans can only fully administer the plan if you do not payroll
deduct the dental plan premium from your employers paycheck. If you chose to
payroll deduct, then you will be required to send in the total amount of our
participating employees premiums each month to us. We can help assist you in
filling out the adding or canceling of employees each month.
If you choose not to payroll deduct, then we can still offer this benefit to
your employees but this will require you to provide a roster of your current
employees and any future hires.
How much can I contribute to
my employees dental plan?
Whatever you feel comfortable in contributing, from 0% all the way up to 100% of
the plans premium. If you choice to contribute all the employees premium, there
would be no need for payroll deduction.
Regardless of whether you choice to partially or fully contribute you will be
able to keep track of active participating employees through our website to
help you determine the total amount of premium you will have to pay Benchmark
Dental Plans each month.
